Grab & go meals start Monday March 30th, 2020. In order to receive this meal,
you must complete this form by 9:30 am the day of the meal.
You can also fill this form out a week at a time.

These meals are for Humphrey Public School children ages 1-18. The pick up times
for meals each day will be 11:30-12:15 with the pick up at the main southeast doors.
No meals will be arranged unless you have filled out this form.
